Maybe directory table is not properly aligned in RAM, or there are errors at SA. 109 alone is OK to rebuild bare ROM, then populate it with ROM modules from any donor of same family and head map.
What is version you need? You can find it inside the end of 109 or in some overlays like 11,12 etc
