The drive is from family Sadle G6

Switched off heads...................... : Yes
Heads map............................... : 0,1,5,7
ROM Firmware version.................... : 00200076

Haven't got the exact one, anyway I have 00200085. Here is 11th file attached. Try my loader if it will work with your drive.
