Identifying SanDisk Flash

Hello everyone,

I'm having some trouble identifying the model # of this SanDisk USB flash drive.
It was brought in without an enclosure and I can't power it up so we need to locate some donor parts.

Attaching some pics of the board.
Any help will be appreciated. :wink:
