If I had to guess, I'd suspect it's a bad NAND on the SSHD. But, perhaps someone else who's seen this specific log message could confirm it.

I've recovered several of these without changing the heads or opening them up.

It is bad NAND. There is no need to open it, but if patch is not working then you need to use donor PCB with same FW revision and swap ROM to it, then 'flush' NAND content to force reloading of patient adaptive data back into donor NAND
