If it ends with LED1A04 error in terminal than you should send LE command to gain access.

Otherwise it can be weak heads or media damage.

Check under the microscope to see if there are any scratches in the SA area on the platter:

If there are no visible scratches under miscroscope than heads are week you should look for the one with this preamp: *PAUNI_L2958*PA VID=0011 PN=000E Rev=0002 to swap.
