From the last SA report, I cant see why this disk is not working. For SPT enter 1311 then check SA again. IF looks the same, check header of mod #11 and verify ROM and SA are same version (to be sure ROM is native).

If nothing helps, then you need to recover data from Hotswap, but if you follow traditional hotswap method of writing all SA to donor I think donor will develop same problem. You can hotswap with just basic adaptive and translator modules copied to donor

There isn't any module 11 in the module list. With hotswap it contains errors.
We're almost sure that Rom is native.

We tried to do traditional hotswap but with no module 11, donor behave as patient.
With basic import, hotswap doesn't work...

If #11 is not displayed then I think is problem with #01 module
